A man from our world waking up in the world of an otome visual novel is a common trope in BL isekai light novels, but there are surprisingly few actual boys love visual novels using isekai plots. Here are a few of the games that do exist.

The most famous isekai BL game is undoubtedly 2022's NU: Carnival, in which you play an ordinary human transported to a magical world, who can level up his allies through sex.  NU: Carnival is a Boys Love RPG featuring battle and exploration elements designed for Android and iOS, though due to its explicit BL content it isn't available through the App Store and instead must be downloaded and installed from its website. It has been a breakaway worldwide hit, registering more than a million downloads within weeks of release. Though developed by a Taiwanese company, it features Japanese voice acting, and is available in English, Chinese, Japanese and Korean.

Laughter Land, from 2006, has never been translated into English, though publisher Langmaor's other titles Enzai and Absolute Obedience were the first BL visual novels ever translated into English. Laughter Land is the story of a villager bored with his ordinary life, who meets a stranger who invites him to a magical world named Laughter Land. He accepts.

2012's Ariard -Shounen Alice- has also never been translated into English, and features an Alice-in-Wonderland-inspired storyline in which the protagonist is transported to Wonderland, told he is the land's queen, and is ordered to choose its new king. 

Stuck in a Yandere Visual Novel... HELP!! is an English-language visual novel from 2022 that makes use of the tried and true isekai plotline of a character getting stuck within a game they've played. The twist here is that all the love interests are yandere, so please pay attention to the content warnings!
